NEW CIIS USERS: After completing CIIS training(s), submit a CIIS User Account Registration Form.
User account requests will be denied if these requirements are not met:

Site has a signed Letter of Agreement (LOA) and Site Administrator Form on file with CIIS.

User has completed required CIIS training(s).

CIIS Site Administrator has approved your user account request.
EXISTING CIIS USERS or ENROLLED SITES: Complete the CIIS User and Site Update Form to request updates to your existing CIIS user account or to update your site’s information in CIIS (change of employment, change of account access type, change of site address, etc.).
