If the Login page is not displaying correctly, press CTRL+F5 to refresh the page. If you continue to experience difficulties, check the following:

  1. You must use a supported browser. WebIZ is supported in:
    • Microsoft Edge
    • Google Chrome
    • Mozilla Firefox


  2. Turn off your browser's Pop-up Blocker.
    • The Pop-up Blocker must be turned off to ensure that WebIZ sends all necessary messages. If you click a button and no window is rendered, check that the Pop-up Blocker is turned off.
    • If the Pop-up Blocker is turned on, a warning message will appear telling you to turn off the Pop-up blocker. Click on the warning message to disable the Pop-up Blocker.


  3. JavaScript must be enabled in your browser. Use your browser’s help function or a Google search to learn how to enable JavaScript in your browser.
*NEW CIIS USERS: Before requesting a CIIS user account, CIIS training is required.

Instructions: Click on the Request User Account link above to create a new CIIS account, change your existing CIIS account, or inactivate a CIIS account.  *You can only complete this form if your site has a signed Letter of Agreement with CIIS.* 
NOTE: All fields marked with * are required. A Training Certificate of Completion needs to be sent to the CIIS Help Desk for your account to be created.
